Grievance Policy

The Sanctuary MD Wellness Center is committed to maintaining an environment in which all individuals are treated with dignity, respect, compassion, and professionalism. Complaints may be directed to: The Sanctuary MD Wellness Center 1808 Roselawn Avenue, Monroe, Louisiana 71201 Phone: (318) 816-4919 Complaints may also be directed to the Louisiana Department of Health. Individuals may submit grievances verbally or in writing. Grievances will be investigated within 72 hours whenever reasonably possible. Retaliation against any person filing a grievance is prohibited. Findings and corrective actions will be documented and communicated to the complainant.
